COMMUNITY HEALTH IMPROVEMENT PLAN REFLECTS COMMUNITY VOICES

Priority areas include access to health and human services, mental health and substance use, and economic and social mobility

Martin County, FL – The Martin County Community Health Advisory Council announces the completion of the Community Health Improvement Plan (CHIP), which identifies goals and strategies for improving health in the county.

The plan identifies three priority areas with actionable goals and strategies:

 

  • Access to Health and Human Services
  • Mental Health and Substance Use
  • Economic and Social Mobility

“The Community Health Improvement Plan is an excellent example of how partners work together to identify issues of concern and set goals to improve health.”  said Carol Ann Vitani, Health Officer for the Florida Department of Health, Martin County. “This collaborative effort helps to make our community stronger, healthier and more resilient.”

The development of this plan began in 2020 with the creation of the Community Health Assessment (CHA) where residents provided feedback related to community health and quality of life. The report also provides extensive county specific health and population data.

The Martin County Advisory Council consists of representatives from more than 25 agencies including the Florida Department of Health, Martin County Government, City of Stuart, Martin County School Board, UF/IFAS Family Nutrition program, the Kane Center, Helping People Succeed, Treasure Coast Food Bank/Whole Child Connection, Mary’s Home, Pentecostal Church of God, Love and Hope in Action (LAHIA), Children’s Services Council of Martin County, House of Hope, Salvation Army, Tykes & Teens, Volunteers in Medicine, Florida Rural Legal Services, Community Foundation of Palm Beach & Martin Counties, The Healing Center of Martin County, Martin County Sheriff’s Department, Light of the World Charities, United Way of Martin County, Healthy Start of Martin County, 2-1-1 Palm Beach/Treasure Coast, Communities Connected for Kids, Project Lift, Christ Fellowship Church, Cleveland Clinic Martin Health, Indian River State College, AmBetter, Martin County Fire Rescue, , Martin County Health and Human Services, Martin County Library System, The Council on Aging of Martin County and the Florida Community Health Center.

The Florida Department of Health in Martin County worked in partnership with the Health Council of Southeast Florida on both the CHA and CHIP.
